It is important to our state association leaders that all of our churches know that they are available and ready to serve our folks in the event that you need help or guidance including information on church revitalization resources. When you click on the Church Request Form, you will access a digital form and upon submission of that form, it will be sent to our state moderator, assistant moderator and state promotional director. After receiving and reviewing the request, the appropriate person will follow up for further discussion.
